This page explains what happens to information you give us through this website. How we handle your health information as a client of the clinic is explained on our Client Privacy page.
Contact form and enquiries
When you use the contact form we collect your name, email address, phone number, the type of enquiry and your message. The form is delivered by Formspree, a form service hosted in the United States, which passes your message to our clinic inbox and keeps a copy for a limited time. We use it only to reply to you. If you would rather not use the form, you can call us on 0411 839 414 or email hello@hearlivewell.com.au.
Website analytics
We use Google Analytics to see how the website is used, for example which pages are visited and how people find us. This information is aggregated and does not identify you. Google may set cookies to do this. You can decline cookies in your browser settings, and the website will still work.
Links to other websites
This website links to other organisations, such as the Hearing Services Program and professional bodies. Their privacy practices are their own.
Client information
Once you become a client, your health information is handled under our Client Privacy Statement, which is available on this website and at the clinic.
Accessing or correcting your information
You can ask to see or correct any information you have given us through this website by contacting us.
Privacy complaints
If you have a concern about how we have handled your information, please contact us first. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner on 1300 363 992 or at www.oaic.gov.au.
